HIPAA Notice of Privacy Practices

This notice describes how medical information about your child may be used and disclosed and how you can get access to this information. Please review it carefully.

Our commitment

Starlight ABA is a healthcare provider subject to the Health Insurance Portability and Accountability Act (HIPAA). We are required by law to maintain the privacy of Protected Health Information (PHI), provide you with this Notice, and follow the terms of the Notice currently in effect.

How we may use and disclose PHI

• Treatment: To provide and coordinate ABA therapy, including sharing information with your child's care team, BCBAs, RBTs, and other treating providers.
• Payment: To verify insurance benefits, obtain prior authorization, and bill insurance carriers or other payers for services rendered.
• Healthcare Operations: Quality assessment, clinical supervision, staff training, and other internal operations necessary to run our practice.
• As required by law: Including mandatory reporting of suspected abuse, public health activities, and lawful court orders.

Uses requiring your written authorization

Most uses and disclosures of psychotherapy notes, marketing communications, and any sale of PHI require your written authorization. You may revoke that authorization at any time, in writing.

Your rights

• Inspect and copy your child's PHI.
• Request amendments to information you believe is inaccurate.
• Request an accounting of disclosures.
• Request restrictions on certain uses and disclosures.
• Request confidential communications at an alternate address or phone.
• Receive a paper copy of this Notice on request.
• File a complaint if you believe your privacy rights have been violated.

Complaints

You may file a complaint with us by contacting our Privacy Officer at privacy@starlightaba.com or (877) 888-8740. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.

Changes to this Notice

We reserve the right to change this Notice and to make the revised Notice effective for all PHI we maintain. The current Notice will always be available on this page.
